Today, we will continue the topic from my last article: 7 Essential Things to Know Before Getting the Best Liposuction in Bangkok Part 1/2 to answer the rest of the questions asked by my patients to get the best liposuction in Bangkok as a solution to a perfect body. 5. Can Liposuction Eliminate Cellulite? Yes or No?

Some patients have unrealistic expectations about liposuction and have misunderstanding that they will have the perfect smooth skin after liposuction, but the answer is ‘NO’. Many patients still get confused between Cellulite and excessive fat which can be treated by liposuction. So today I will clarify what cellulite is and why it can’t be treated successfully by any kind of liposuction.

Cellulite is the term describes pockets of fat deposits surrounded by fibrous connective tissue just below the skin surface. When the fat cells grow, the compartments holding those fat cells get crowded, the fibrous septae pull down on the skin creating uneven surface of cellulite. It makes the skin surface puckered, lumpy and dimpled, commonly found in thighs, hips, buttocks and abdomen. Approximately 80-90% of women develop cellulite at some point after puberty. Only 10% of men develop cellulite.

Grading of cellulite is shown in below photos. While primary cause of cellulite is still inconclusive, weight gain, hormone, genetics, aging process, smoking, dietary and lifestyle are found to play important roles. If liposuction had the ability to completely remove cellulite, it would be extremely desirable due to its dual advantage of eliminating both fat and cellulite. Regrettably, that is just NOT TURE.

Information from American Academy of Dermatology association. Cellulite treatment: what really works? said that It’s important to note that the procedures that just remove fat often have no impact on cellulite. It needs specialized procedures to change its appearance .

However, this does not imply that liposuction is not a helpful operation in plastic surgery. Liposuction is widely esteemed among plastic surgery patients due to its ability to reshape and re-contour their bodies, and perhaps enhance, but not entirely remove the visibility of cellulite as it’s fat pockets are located in the upper part of skin closed to dermis layers while all kinds of liposuction can remove fat only in deeper subcutaneous layer.

From a personal standpoint, it is not advisable to use conventional liposuction for eliminating cellulite since it might accentuate the appearance of cellulite. Since conventional Liposuction is an option to eliminate unwanted excessive fat in deep subcutaneous layer only!

6. Conventional Liposuction in Small Areas. Is it too painful?

Liposuction may be limited in its ability to remove extra fat from the small curving area under the chin and jawlines due to its potential unsatisfied results caused by the large and rigid metal tip used during the process, such as mechanical trauma, nerve damage, and bruising. And there is higher possibility to have rippling skin on treated area which is the most unwanted result! Moreover, using rigid metal tip can cause intolerable pain during the procedure. Thus, in order to get a more defined and chiseled neck and jawline with firmer and smoother skin, alternate laser methods are recommended.

7. Can I have Conventional Liposuction safely under Local Anesthesia ?

Most patients believe that procedures under local anesthesia is safe and easy to carry on but actually it is NOT entirely TRUE. Every board-certified plastic surgeon including me considers liposuction as a major operation that must be done under general anesthesia regarding to international standard for patients’ highest safety.  
As conventional liposuction is not able to melt fat into oil; therefore, we need to cause mechanical trauma for fat cells to be dissolved and suctioned out. 

A large metal tip must be used to create such trauma, resulting unbearable pain during the procedure. Thus, if local anesthesia is used in this scenario, a lot amount of lidocaine  must be used to relieve such pain. Such amount of lidocaine may cause lidocaine intoxication which could lead to life threatening condition in some cases.

That’s why board-certified plastic surgeons choose general anesthesia instead to avoid this condition. It is also easier for the surgeon to perform the  liposuction procedure  since they do not have to ensure total numbness of the operative region. The primary advantage for patients is that it enables the completion of more extensive procedures in a single operation. So higher amount of fat can be eliminate during conventional liposuction under general anesthesia . That means patients can be slimmer under GA compared to local anesthesia . Moreover , if the idea of being conscious during surgery is intimidating, general anesthesia may be a more desirable option.

However, there are also sometimes where I find that some patients have limitations to be under general anesthesia due to health concerns. So, every time before liposuction, a physical check-up should be done to ensure patients’ safety.
